Know your peptide,
know your number.

Milligrams don't mean the same thing across peptides. Milliliters don't either. Here is what actually changes, and why it matters most the day you draw your own syringe.

Milligrams don't mean the same thing across peptides, and milliliters don't either. Semaglutide, tirzepatide, and retatrutide have different dosing ceilings and different half-lives. Twelve milligrams of retatrutide is a real, studied dose. Twelve milligrams of semaglutide would be five times its approved ceiling. A dose number without the peptide name attached is close to meaningless.

It gets sharper with compounded vials. A brand pen does the concentration math for you. A vial makes you do it yourself, and two vials with the same volume can carry very different amounts of drug. If you draw your own syringe, the one number you cannot skip is your vial's concentration, its milligrams per milliliter.

For years, obesity medicine was simple in one narrow way: you got a pen, you clicked it, you got your dose. The pen did the thinking. As more people move to compounded formulations drawn from vials, that quiet convenience disappears, and a whole layer of math that used to be invisible is suddenly the patient's job. This is a plain-English walk through that math, so the numbers you hear in a forum or from a friend actually mean something when you hold your own syringe.

A dose number is meaningless without the peptide's name

These three drugs get compared by raw milligram number constantly, and that comparison is a trap. They have different potencies, different mechanisms, and different ceilings. The same number means something completely different depending on which molecule it is attached to.

Look at the ceilings. On semaglutide, 2.4 mg a week is the top approved dose for weight management. On retatrutide, 12 mg a week is a real dose studied all the way through a Phase 2 trial, the arm that produced the largest average weight loss. So when someone says "I'm on twelve," the number tells you almost nothing until you know which of these three they mean. On one drug it is the maximum; on another it would be roughly five times the maximum. Retatrutide is also not FDA approved as of this writing, it remains investigational.

The half-lives matter too, though more quietly. All three sit in the once-weekly range, six to seven days, which is why a single missed day does not send your levels off a cliff. But that is a property of the molecule, engineered on purpose, not a license to improvise a schedule.

Milligrams, milliliters, and the one that trips everyone up

This is the heart of it, and it is the single most common place people get confused. Three terms, and they are not interchangeable.

Milligrams. The actual amount of medication. This is your real dose, the thing that acts on your body.

Milliliters. The volume of fluid you draw into the syringe. This is just how much liquid, not how much drug.

Milligrams per milliliter. How concentrated the vial is. The bridge between the two above, and it changes vial to vial.

Here is why the third one is the whole ballgame. The same mark on your syringe, say a half a milliliter, delivers a completely different actual dose depending on how concentrated the vial is. Volume drawn is not dose. Dose is volume times concentration. Miss that, and you can be certain you are taking one amount while you are actually taking double, or half.

The same syringe mark, two different doses

Most insulin-style syringes are marked in units, not milliliters. On a standard U-100 syringe, 100 units equals 1 mL, so the 50-unit line is 0.5 mL. That is the mark you actually read on the barrel.

Draw to 50 units (0.5 mL) from a vial concentrated at 5 mg/mL, and you have drawn 2.5 mg of drug.

Draw to that exact same 50-unit line from a vial at 10 mg/mL, and you have drawn 5 mg, double the dose, same line on the syringe.

Nothing looked different. The syringe read 50 units both times. Only the vial's concentration changed, and it silently doubled the dose. That is the entire reason a unit count or a milligram number borrowed from someone else, on a different vial, can be dangerous. Units tell you how far you pulled the plunger. They do not tell you how much drug came with it.

Brand pens versus compounded vials

Brand pens, Wegovy, Zepbound, Mounjaro, are pre-measured. The concentration math has already been done and locked in at the factory. You click the pen, you get the labeled dose, and there is no calculation for you to make or get wrong. That is a real safety feature, not just convenience.

Compounded vials are a different system entirely. The dose is expressed as a concentration, milligrams per milliliter, and that concentration varies from bottle to bottle and pharmacy to pharmacy. A vial's strength is a property of that specific vial. This is why the mg-versus-mL math above stops being trivia and becomes the thing standing between you and a dosing error. The moment you draw your own syringe, the pen's invisible math becomes your visible responsibility.

Why dose increases deserve real respect

People sometimes talk about dose escalation as if the only cost of going higher too fast is a rough couple of days of nausea. The clinical record says the stakes can be higher than discomfort.

A documented case

A published case report describes a woman on the top approved weekly tirzepatide dose who, after a dose increase, developed severe gastrointestinal symptoms so profound they drove her electrolytes to life-threatening levels and triggered a cardiac emergency requiring resuscitation. This happened at standard weekly dosing at the top tier, not on any experimental schedule.

The lesson is not fear, it is respect for the escalation itself. Any time a dose goes up, gastrointestinal side effects can move from uncomfortable to genuinely dangerous, because sustained vomiting or diarrhea can strip out the electrolytes your heart depends on. That is the real reason the standard protocols escalate slowly, and the real reason a dose increase is a conversation to have with your prescriber, not a milestone to rush.

The one habit worth building

If this whole piece collapses into a single practice, it is this: when you say your dose out loud, say the peptide with it. Not "I'm on twelve." Say "I'm on twelve of retatrutide," or "I'm on five of tirzepatide." The number alone invites a bad comparison. The number with the peptide attached is actual information.

And if you draw from a vial, add one more habit: know your concentration before you know your dose. Your milligrams per milliliter is the fact that makes every other number true or false. Confirm it against your own vial and your own prescriber, never against a number you heard secondhand from someone whose vial you have never seen.

Cliff's Note

I've walked the whole path this article describes. I started on semaglutide in a fixed-dose pen, moved to tirzepatide in a pen, and over the last year moved to compounded vials of both tirzepatide and retatrutide, drawing my own syringe. That last move is exactly where the mg-versus-mL math stopped being abstract and became something I had to get right every week.

I titrated retatrutide up to 12 mg, and at the top I started noticing side effects that outweighed the extra benefit. So I brought it back down to 5 mg a week. I'm still losing weight slowly at 5, my food noise is manageable, and I'm hitting my protein goals most days. The number that was "best" on paper wasn't the best number for me, and knowing exactly what I was drawing is what let me make that call with my provider instead of guessing.
