A Dual Agonist, One Step Further

Tirzepatide is a dual agonist, it activates two hormone receptors simultaneously rather than one. It targets the same GLP-1 receptor as semaglutide, but adds the GIP receptor as a second pathway. That combination produced meaningfully better weight loss than GLP-1 alone.

FDA-approved as Mounjaro (type 2 diabetes) and Zepbound (chronic weight management), both manufactured by Eli Lilly. A direct head-to-head clinical trial against semaglutide, SURMOUNT-5, confirmed tirzepatide's advantage for sustained weight loss.[2]

We found tirzepatide the hard way, in a Portuguese pharmacy during a trip in March 2025, after a year without any GLP-1 coverage. The results came back fast. I hit a new all-time low. The difference between semaglutide and tirzepatide was real and noticeable. Not dramatically different day to day, but the trajectory was steeper and more consistent. If you've plateaued on semaglutide, tirzepatide is absolutely worth understanding.
